Tracking of TPC HV trips

As per the discussion at the TPC meeting last week, Dmitry has helped
create a tpcAnodeHV table in the offline DB as outlined by my request:

I have placed initial values into the table of 1170V for inner sectors
and 1390 for outer:

Joe has identified 99 potential trips, but we need to verify these are
really trips.
